Guardianship Referrals
To be eligible for guardianship, an individual must be:

18 years old or older;
-
Adjudicated incapacitated;

Subject to the jurisdiction of a participating court; and
-
Have no other viable alternative for guardian.

To ensure proper organization and efficiency, we ask that the referral form be filled out completely and submitted along with a physician’s report. Without both documents, an individual’s place on the waiting list will not be secured. If the referred person lives in LaPorte County, please email the Physician's Report to Kayla Shifley at kayla@inguardian.org. If the referred person lives in Porter County, please email the Physician's Report to Kardetra Redmond at kardetra@inguardian.org.
Physician's Reports

Please note that an application will not be reviewed without the Physician's Report as Indiana Guardianship Services, Inc. uses this to determine appropriateness for services and petitioning for guardianship. Pending where the individual resides, the LaPorte County Physician's Report or the Porter County Physician's Report will be used. Indiana Code 29-3-1-7.5 is included to assist a physician in determining if an individual meets the State of Indiana's criteria for an incapacitated person. This form may be found here.
